Salary £24,736.92 per annum

37.5 hours per week 

 

Turning Tides mission is to end local homelessness in Worthing, Adur, Littlehampton, Horsham and Mid Sussex. We have over 100 staff and 300 volunteers.

The Hospital Admission Reduction Pathway (HARP) Navigator is an exciting new role as part of the West Sussex HARP. This has been funded by Public Health England as part of the Rough Sleeping Grant Programme. This is a rare opportunity to support the implementation and development of a pioneering new service.

It is recognised that rough sleepers across West Sussex have unmet physical and mental health needs which can often cause difficulties when trying to engage with services in a productive and long-term arrangement. There will be three HARP Navigator positions available in total. We are currently recruiting for the Turning Tides post but please do state in application if you are interested in any other posts/locations below:

Turning Tides – 12 months starting in February 2020 based in Worthing

Stonepillow- 10 months starting in April 2020 based in Chichester

Crawley Open House – 9 months starting in May 2020 based in Crawley

Your role will lead on coordinating and supporting people who are homeless and accessing Worthing Accident and Emergency services, working closely with the senior medical team, alcohol nurse and Psychiatric Liaison Team. Your role will also connect those individuals with other community services including the HARP Clinical Navigator and peer mentors upon discharge. You will have strong communication and negotiating skills, to be able to identify a range of solutions for each individuals, and the ability to work in a fast paced environment with a commitment to helping individuals to make positive changes in their lives.

We are looking for individuals who are dynamic and have experience of working with vulnerable people and the difficulties they experience in accessing services. You will also have experience of supporting these individuals to address their needs through effective case work management.

You will be passionate about working for a charity that is striving for communities without homelessness.

You must be able to work independently and as part of a team. Ability to manage and plan your own workload and meet deadlines and targets is essential. Good IT Skills and in particular Excel skills are a must.
